Tolerability of Tadalafil
For Tadalafil, generally mild and self-limited adverse reactions were reported.
- The most common side effects of Tadalafil are: headaches, indigestion, back pain, muscle aches, flushing, and a stuffy or a runny nose.
Severe side effects that are a signal for stopping immediately the administration of Tadalafil are the following: allergic reactions including rashes, chest pain, priapism, an erection that lasts more than 4 hours, sudden loss of vision in one or both eyes, which could be a sign of non-arthritic anterior ischemic optic neuropathy (NAION), sudden decrease or loss of hearing (sometimes with ringing in the ears and dizziness) - Uncommon side effects (reported in 1 to 10 in every 10000 patients) include: dizziness, stomach ache, feeling sick, being sick (vomiting), reflux, blurred vision, eye pain, difficulty in breathing, presence of blood in urine, prolonged erection, pounding heartbeat sensation, a fast heart rate, high blood pressure, low blood pressure, nose bleeds, ringing in the ears, swelling of the hands, feet or ankles and feeling tired.
- Rare side effects (reported in 1 to 10 in every 1000 patients) that have been reported are: fainting, seizures and passing memory loss, swelling of the eyelids, red eyes, sudden decrease or loss of hearing, itchy red welts on the surface of the skin, penile bleeding, presence of blood in semen and increased sweating.
The tolerability of Tadalafil must be evaluated when choosing a dose regimen. Usually, the prescribed once-a-day dose is 5 mg, but in the unlikely event that some unpleasant reactions occur, the dose must be down titrated to 2,5 mg.
The same applies for the as needed dose. You should start with the 10 mg regimen, and use a lower dose if you feel uncomfortable. Or, you can increase the dose to 20 mg based upon the efficacy and tolerability.
Which drugs could influence the action of Tadalafil?
Tadalafil is contraindicated to be administrated when you are suffering from angina pectoris and you are administering any form of organic nitrates (nitroglycerine, isosorbide dinitrate, isosorbide mononitrate, or nitroprusside), because it increases the hypotensive effect of the nitrates. Using Tadalafil with this type of drugs may cause an excessive decrease of blood pressure and finally, cardiovascular colaps.
Tadalafil co-administrated with alpha androgenic blocking agents (doxazosin, tamsulosin, afluzosin) is not recommended because both drugs are vasodilators with blood pressure lowering effect.
Hypotensive effects of riociguat, a drug used to treat pulmonary arterial hypertension and chronic thromboembolic pulmonary hypertension, have been shown to be increased when administering Tadalafil.
Other drugs may influence the pharmacokinetic properties of Tadalafil
Antiacids modify the rate of absorbtion;
Therapeutic effects and side effects could be increased by inhibitors of CYP3A4, the enzyme involved in Tadalafil metabolism, such as ketoconazole or HIV protease inhibitors;
The antibiotic rifampicin, a CYP3A4 inducer reduces the effects of Tadalafil.
Tadalafil is contraindicated to be concomitantly taken with Cialis, because both drugs contain the same active substance and their concomitant use might lead to an overdose.
Patients with cardiac disorders should be very carefully when taking Tadalafil. They should report to the physician or pharmacist all the heart medicines they are taking for a proper evaluation of the risk/benefit ratio.
