Is There an As-Needed Version of Cialis?

Yes. The original Cialis formulation won MHRA approval in 2003 and was designed to be used on an as-needed basis. In that respect, it was like Viagra and Levitra, both of which won MHRA approval before Cialis. However, Cialis offered a far longer lasting duration of action — up to 36 hours — that earned it the nickname of the “weekend drug.” This was far longer than the four to six hours offered by Viagra and Levitra.

How Does Daily Cialis Work?

Cialis is a member of a family of drugs known as PDE5 enzyme inhibitors, that are indicated as the primary treatment for erectile dysfunction.
Cialis blocks an enzyme that causes an erection to go flaccid. Although Cialis is similar to Levitra and Viagra, its dose, an onset of action, and duration of action is unique.
Cialis was developed by Icos, a small American biotech company located in Washington, USA.

When a man gets sexually stimulated, a chain reaction occurs in the tissue of the penis that results in elevated levels of a substance called cyclic guanosine monophosphate (cGMP).
As long as there are sufficient levels of cGMP, the penis can remain erect. The more cGMP, the more robust and durable the erection.
If you inhibit the degradation of cGMP it stays around in the penis longer, producing a more durable erection.
Cialis effect is that it preserves the elevated levels of cGMP that are created when a man is sexually stimulated for a stronger, more durable erection.
Cialis blocks the enzyme phosphodiesterase-5 (PDE5) which is responsible for the neutralization of cGMP. Cialis (tadalafil) is a highly selective inhibitor of PDE5.
PDE5 is not the only phosphodiesterase involved in the erectile physiology but it appears to be the most important one.
The nice thing about Cialis is that it won’t cause erections at inappropriate times because it only blocks degradation of cGMP produced in reaction to sexual stimulation,

 

Are Any Drugs Incompatible with Cialis?

Talk to your doctor about all the medicines you take, especially:
– medicines called “nitrates” which are often prescribed for chest pain
– medicines called guanylate cyclase stimulators, such as riociguat (Adempas®), used to treat pulmonary arterial hypertension and chronic-thromboembolic hypertension
– alpha-blockers often prescribed for prostate problems
– blood pressure medications
– medicines for HIV or some types of oral antifungal medications
– some types of antibiotics such as clarithromycin, telithromycin, erythromycin (several brand names exist, please contact your healthcare provider to determine if you are taking this medicine)
– other medicines or treatments for erectile dysfunction

Clinical studies show that alcohol concentrations were not affected by coadministration with tadalafil taken in 10 mg or 20 mg doses. Tadalafil concentrations were not influenced after co-administration with alcohol.
